Assess runs a private nursing home. Assess applied state pollution control board for the purpose of NOC by CA  certificate.
Application enclosed by CA certificate
CA,s certificate regarding un-depreciated value of the fixed assest of the industry at the end of last finicial year.
Any format of certified by CA for NOC to state pollution control board.

Before carrying out any action at industry, plant, establishment or project, the entrepreneur is required to obtain CTO (consent to operate)/NOC under the relevant pollution act, form the State Pollution Control Board. Every state has its own procedure to obtain NOC/CTO.  
Apart from various other documents required to be provided to the State Pollution Control Board along with application, One more important document is required which is “CA’s certificate regarding un-depreciated value of the fixed assets of the industry at the end of last financial year.”
There is no fixed format for this CA’s certificate. But standard procedure should be followed as per Guidance note on audit of fixed assets and it may be prepared on letter head and it may include list of fixed assets, As on date. value of each assets, name of Company and a note regarding the certification work like other certificates, say, CA certificate about investment in Plant & machinery etc.
